Enrollment Instructions
When a new User is ready to enroll in The PHD Network, reviewing the components of The PHD Network will provide each new User with a basic understanding of the program and how the User can benefit from using The PHD Network. This can help increase User participation in the program.
The Registration Page allows the User to set up a PHD Network account. If Registration Code / Sponsor Code field is present and the User is part of a Sponsor’s group, the Registration Code (Sponsor Code) provided by the Sponsor is entered on this page in the field provided. If the User is not part of a Sponsor’s group, he/she should leave the Registration Code blank and complete the remaining information and the Personal Health Assessment. The User will be prompted to provide payment in the form of a credit card prior to entering The PHD Network.
The Registration Page collects a variety of information. Only the items in red are required. On this page the User also creates the personal Username and Password that he/she will use to access The PHD Network in the future. Once the information on the Registration Page has been submitted, the User’s account is created, regardless of whether or not the Personal Health Assessment is completed. If a User does not complete the Personal Health Assessment during the initial set up, upon returning to The PHD Network the User should log into the The PHD Network as a current member. The User will be directed to the Personal Menu Page. (Note: There will be no lessons assigned until the Personal Health Assessment is completed.) From the Personal Menu Page, select Personal Health Assessment from the Menu and complete to receive lessons.
